From Feelings of Insecurity to Feeling Confident: The Gynaecomastia Solution

At Cosmetic Surgery Zurich we find it crucial to provide our clients with comprehensive information. This time we discuss a problem affecting no less than 40% of men: gynaecomastia. This condition, characterized by the enlargement of male breast tissue, can cause significant physical and emotional distress.

This guide aims to educate on the causes, treatment options, and the transformative impact of tackling gynaecomastia. To empower all men suffering from this condition!

What is Gynaecomastia?

Gynaecomastia refers to the development of excess glandular breast tissue in males, leading to breast enlargement and often a feminine appearance of the chest. It can occur in one or both breasts and may affect men of any age. While it is not a life-threatening condition, gynaecomastia can profoundly impact self-esteem and quality of life.

Causes of Gynaecomastia

Hormonal fluctuations, particularly an imbalance of estrogen and testosterone, play a significant role in the development of the condition. This imbalance can occur temporarily during puberty, leading to breast enlargement in adolescent boys. In 75% of the cases the enlargement will disappear as soon as the hormones settle, at the end of puberty. However, in the remaining men, the gynaecomastia persists into adulthood, posing challenges for the affected individuals.

Medications and substances that alter hormone levels or have estrogen-like effects can also result in gynaecomastia. These may include anabolic steroids, certain antidepressants, anti-androgens (often taken against hair loss!), and recreational drugs like marijuana and heroin.

Additionally, gynaecomastia can be a symptom of underlying health conditions such as liver disease, kidney failure, and thyroid disorders, but also of a tumour in breast or testicle. Therefore a newly developing gynaecomastia, especially after puberty, calls for medical evaluation (a few quick and easy tests will provide clarity, usually a blood test + ultrasound).

Lastly, there’s pseudo-gynaecomastia, that is part of being overweight. The excess breast tissue does not consist of excess glandular tissue but mainly of fat tissue. Unfortunately, even in case of subsequent weight loss, the breasts often stay enlarged.

The Psychological Impact

Both ‘real’ gynaecomastia and pseudo-gynaecomastia can have profound psychological effects, causing feelings of embarrassment, self-consciousness, and social withdrawal. Men with gynaecomastia often avoid certain activities such as swimming or going shirtless, impacting their quality of life and self-image. Addressing the physical aspects of gynaecomastia through cosmetic surgery can significantly improve self-esteem and restore confidence.

Treatment Options

The specific treatment will depend on the severity of the condition, underlying causes, and individual preferences. As said before, breast enlargement that happens during puberty has a good chance of resolving spontaneously. Hormonal therapy can be a solution in case continuous hormonal problems are the underlying cause. Obviously it is crucial to refrain from anabolic steroids and marijuana (for your health in general too!). And adopting a healthy lifestyle, including regular exercise and a balanced diet, can help manage gynaecomastia in some men.

However, over time the enlarged breast tissue tends to turn fibrotic. Meaning, if the (pseudo-)gynaecomastia exists for more than one to two years, it is unlikely to ever disappear spontaneously. In these cases, cosmetic surgery offers the solution to return to a more masculine chest contour. The options include:

  1. Excision: in case of enlarged glandular tissue, excision surgery may be necessary. This procedure removes excess breast tissue and sometimes excess skin, reshaping the chest for a more masculine appearance.
  2. Liposuction: in case of a pseudo-gynaecomastia, liposuction can be an effective minimally invasive solution. This procedure involves the removal of fat tissue through small incisions, resulting in a smoother, more contoured chest. The area that can be treated by liposuction is larger than by excision only.
  3. Combination of the two methods: some men benefit from an excision of the glandular tissue plus a liposuction of the surrounding fat tissue.

These operations take place under general anaesthesia, without overnight-stay.
